Pet Requirements Prior to Boarding

Pet has had Rabies, DHLPP, and Bordetella vaccinations within 12 months of the boarding date.
Provide your Veterinarian’s name and telephone number, so we can verify your pet’s vaccination information.

WE DO NOT ACCEPT PITBULL, AMERICAN BULLDOG, STAFFORDSHIRE TERRIERS, OR CHOW BREEDS
All dogs must wear a regular buckle-on or snap-on collar with their own leash
Dogs may be brought in wearing choke chains, constricting collars, or harnesses, but they cannot wear them once they are checked into Pampered Pets
Provide us information on your dog’s overall health, any health problems, or behavioral issues
Provide a list of your dog’s medications and a full description of when medications are to be given

Pet has had Rabies, FVRCP, and Feline Leukemia vaccinations within 12 months of the boarding date
Provide your Veterinarian’s name and telephone number, so we can verify your pet’s vaccination information

WE DO NOT ACCEPT DIABETIC CATS
All cats 7 months old or older must be spayed or neutered
Cat(s) must be brought in using a carrier. We will store your carrier for you
Provide us information on your cat’s overall health, any health problems, or behavioral issues
Provide a list of your cat’s medications and a full description of when medications are to be given
